Tracey

Tracey’s story demonstrates how consistent support, skill-building, and determination can empower individuals to achieve independence, stability, and improved wellbeing.

Tracey was referred to Key and moved into an outreach-supported property where she began receiving weekly support. During her time there, she developed key independent living skills such as cooking, cleaning, and budgeting, with structured support and encouragement from staff. She also worked closely with her Community Psychiatric Nurse (CPN), which played an important role in improving her mental health and confidence.

As Tracey’s skills and self-assurance grew, she expressed a desire to move on from supported accommodation. Our team supported her through this transition, helping her apply for a general-needs tenancy and place bids on suitable properties. A phased approach, focussed on gradually reducing support, helps individuals like Tracey move on from supported living to full independence while building confidence and lasting stability.

Tracey successfully moved into her own tenancy, where she quickly began putting her independent living skills into practice. She has continued to maintain her property, manage her finances responsibly, and meet her rent commitments. Tracey also remains engaged with mental health services and her CPN, ensuring she has ongoing support when needed.

Her support with Key Community was brought to a successful close after she demonstrated confidence, stability, and the ability to manage life independently. Tracey’s journey is a powerful example of how consistent support and personal determination can lead to lasting independence and wellbeing.
